3. Consultation Requests
This Site provides a link to the Precision Vitality patient portal, operated by Cerbo, our electronic health record platform. When you click the consultation request link, you are redirected to Cerbo's secure, HIPAA-compliant platform.
Submitting a consultation request through the Cerbo portal does not guarantee acceptance as a patient, establish a provider-patient relationship, or constitute a medical appointment. Consultation requests are reviewed by our clinical team and we will contact you to confirm eligibility and scheduling.
4. Telehealth Services
Precision Vitality offers telehealth services to patients in California. Telehealth services are provided subject to a separate patient agreement and are available only to established patients who have completed our intake process.
Telehealth services are governed by California law, including the Medical Practice Act and applicable telehealth regulations. By requesting telehealth services, you acknowledge that:
- You are physically located in California at the time of your telehealth visit, unless otherwise permitted by applicable law.
- Telehealth services may not be appropriate for all conditions or situations.
- Technical issues may occasionally interrupt or prevent telehealth visits, and Precision Vitality is not liable for service interruptions beyond our reasonable control.
- You have the right to request an in-person visit as an alternative to telehealth, where clinically appropriate.
5. Scope of Practice and Services
Precision Vitality's services are provided by a licensed nurse practitioner in the State of California. Our services are limited to the scope of practice authorized under California law for nurse practitioners.
Services offered by Precision Vitality may include psychiatric medication management, integrative and functional medicine consultations, peptide-based therapies, and hormone optimization treatments. Not all services described on this Site may be available to all patients. Availability of specific treatments is subject to clinical assessment, applicable law, and regulatory requirements at the time of service.
