Fee For Service
Billing for fees and costs
Precision Vitality Nursing Corporation operates on a fee-for-service, cash-pay basis. We do not bill insurance, Medicare, or Medi-Cal. This model allows us to focus entirely on you — your time with your provider is not dictated by insurance limitations or prior authorizations. Fees will be reviewed at the end of your consultation and must be paid in full or services may be disrupted per our financial policy. Some plans may include laboratory testing, but most plans do not cover laboratory testing or supplements. Options for in-home or office lab draws are available for an additional fee from the companies we contract with. We are not responsible for these added fees and the contract will be between the patient and the phlebotomy company. Compounded medications are included for select programs and this will be reviewed and payment will be collected prior to a payment being made. A 10% discount is offered on all supplements and products ordered through Fullscripts.
Payment
Payment is due on the day of service. We accept all major credit and debit cards, processed securely through Propeller within your HIPAA-compliant electronic health record.
Cancellation Policy
We ask that appointments be cancelled or rescheduled at least 24 hours in advance of scheduled appointment. Late cancellations and no-shows are subject to a $100.00 missed appointment fee.
Superbills
Superbills are available upon request for psychiatric appointments. While we do not bill your insurance directly, a superbill may allow you to seek potential reimbursement from your insurance provider independently, depending on your plan and out-of-network benefits. We do not have any control over reimbursement but supply this as a courtesy for you to attempt to seek partial reimbursement.
Insurance & Medicare Disclosure
Precision Vitality Professional Nursing Corporation is not in-network with any insurance carrier and does not participate in Medicare or Medi-Cal. We operate exclusively on a fee-for-service, cash-pay basis.
Medicare-Eligible Patients
Precision Vitality does not participate in Medicare. Under federal law, Medicare-eligible patients who wish to receive care at our practice are required to enter into a private contract with their provider for services rendered on a fee-for-service basis. Services provided under this private contract are not reimbursable by Medicare.
By signing your treatment contract, you agree that neither you nor Precision Vitality will submit a claim to Medicare for any services rendered at our practice — even if those services would otherwise qualify for Medicare coverage.
Please note that this private contract applies only to Precision Vitality Professional Nursing Corporation and your treating provider. It does not affect your Medicare benefits with any other physician or healthcare provider. You remain free to receive Medicare-covered services from any other provider who participates in Medicare.
